This is back when I could actually keep Jonny-boy company without giving him a cold ;-P Right now he has the colostomy bag on the side that's facing us in this picture, but it's mostly inside his diaper. He also is considerably more bundled up, so his hands aren't laying out like this. He likes them stretched out, but then he pulls on the respirator; he also likes being bundled, and at the moment that's keeping him quieter than letting him grab whatever he wants.
